Framing the Initial Problem
How can we develop a Shared Decision-Making Tool (SDM) that empowers patients and their care teams to make educated decisions about removing lines?
Starting by creating a visual understanding of the journey that transplant patients go on, and to aid our secondary research efforts. We developed a journey map and infographic to make sure that we understand the ins and outs of living with a CVC line.
Explore and Create
From our understanding of what a good SDM does, we conducted further research on what content is a best fit in our tool by interviewing clinical teams and caregivers in the network. Using this, we explored the first concepts of the tool in paper format.
Refinement
Through real use-testing feedback from patients, families, and providers. Alongside internal role-play techniques and examination, these key findings take us into rapid iterations and turnarounds into a digital format.

Final Framing and Deliverables
From our learnings, the tool shifted into an informed decision-making application where the ambiguous section of the patient's removal journey is highlighted.
This fundamentally allows the patient and care team to have their own interdependent functions to then facilitate partnered communication regarding removal time.
